Transforming Lives Through Health

What social work support looks like

Social work is about more than services or systems; it’s fundamentally about people. Our social workers are dedicated to supporting your health and wellness as you navigate life’s challenges, especially when things feel overwhelming, confusing, or difficult to handle alone. This support may encompass emotional assistance, advocacy, practical problem-solving, or connecting you with essential mental health services. We prioritize what matters most to you.

How we can support you

Emotional and wellbeing support through health and wellness initiatives. Navigating the NDIS and other support systems with the help of mental health services. Advocacy when you need your voice heard, especially from social work professionals. Support during life transitions or crisis situations, connecting you with housing, health, or community services. Supporting families and carers in their journey. Building confidence, resilience, and stability for a healthier future.

NDIS social work support

If you’re an NDIS participant, social work can play an important role in enhancing your health and wellness by helping you understand and effectively utilize your plan.

We can support you to:

- Navigate services and providers

- Build supports around your goals

- Strengthen independence and daily living

- Coordinate with other allied health professionals

- Access community and informal supports, including mental health services

We keep things simple, practical, and focused on your goals.

Support for families and carers

Caring for someone can be meaningful and also incredibly demanding. We support families and carers by focusing on their health and wellness through: Providing emotional support and guidance, which is vital for mental health services; Helping manage stress and burnout effectively; Offering practical strategies and information tailored to their needs; and Supporting communication and family wellbeing through social work initiatives.

Our approach to care

Our social work practice prioritizes health and wellness by being:

- Person-centred – your goals guide all our mental health services.

- Strengths-based – we focus on what’s working in your life, not just the challenges.

- Trauma-informed – we prioritise safety, trust, and choice to ensure your well-being.

- Practical and real-world focused – we provide support that integrates seamlessly into your everyday life.

We meet you where you are, without judgement.

When to reach out

You might benefit from social work support, particularly if you are:

- Feeling overwhelmed or stuck

- Navigating the NDIS or other complex systems

- Experiencing changes in housing, health, or family circumstances

- Supporting someone with additional needs

- In need of advocacy or guidance regarding mental health services

If it feels like too much to manage alone, reaching out for help is a crucial step toward achieving overall health and wellness.
